  •  CPR- American Heart Assoc (only)

  • Documentation of shot records / titers are required for various clients

  •  Documentation of negative TB results (within past 12 months)

  •  Statement of Good Health (physical)

  •  Two forms of official identification

  •  Current and unrestricted professional license
